l. The undersigned swears and affirms, under the penalties for perjury, that he/shc has good cause to believe that:

Sometime during the period of time between the dates of 201 1, and 2014 in the County of Allen and in the State of lndiana
said Defendant did knowingly commit the crime(s) of: DATE RANGE 11/3/20] 1-1 1/2/2013 Ct 1, Sexual Misconduct with a
Minor, Class Felony (deviate sexual conduct/oral to defendant) and Ct 11, Sexual Misconduct with a Minor. Class Felony
(touch/fondling); DATE RANGE 1/22/2011-11/2/2011 Ct 111, Child Molesting, Class Felony (touch/fondling): DATE
RANGE 1/22/20] 1/2/2013 Ct 1V, Vicarious Sexual Grati?cation, Class Felony and Ct V, Dissemination of Matter
Harmful to Minors, Class Felony.

5.1.1

The A?ian't, Detective Deionzo Myles FWPDI 674, is an of?cer of the Fort wayne Police Department. The Aff ant believes
and has good cause to believe, that the said Defendant committed the aforesaid offense(s). The Af?ant bases his/her belief
on the fact that sometime during the period of time betwee the dates of 201 l, and 2014 the defendant did knowingly and
intentionally commit aforesaid crime(s) to 100 (MW 1 wit:

4. While the defendant was a? teacher at the Imagine Master Academy during the years 201 and 2013, he committed Ct 1.
Sexual Misconduct with a Minor, .Class Felony (deviate sexual conduct/oral to defendant), and Ct 11, Sexual Misconduct
with a Minor, Class Felony (touch/fondling) to theivictim whom was at the time, approximately 13 years of age and in the
6'11 grade. The defendant had the victim perform oral sex on him several times=in his class room during the aforementioned

  
 
   
  

time frame.

5. The defendant committed Ct 111, Child Molesting, Class Felony (touch/fondling) between the dates-of January 22. 201 
and November 2, 2011 when he touched the victim's breasts on her skin with his hands and sucked the victim?s breasts. The
defendant also touched the victim?s vagina with his hand on her skin.

6. The defendant committed Ct 1V, Vicarious Sexual Grati?cation, Class Felony, and Ct V, Dissemination of Matter Harmful
to Minors, Class Felony during January 22, 2011 and November 2, 20 13. The defendant committed the Vicarious Sexual
Grati?cation when he watched the victim on video chat masturbate herself for him several times. The defendant committed
the Dissemination of Matter Harmful to Minors when he made the victim perform oral sex on him and masturbate him
several times.

7. The defendant committed the aforesaid crime(s) in Fort Wayne/Allen Coun .
