CS Jennifer White entered for duty at Unit 29 Building, Officer Shalanda Payne, and Barbra Johnson was on duty from the night shift. At that time 1" CS White heard an Offender Willitnn Nelson it 67533 mumbling obscene language as he was standing in A-Zone door. At that time Of?cer Shalanda Payne asked him if he wanted to come in the hallway sign his RVR he stated ?(yes l?m going to sign was wall-ting away and he turned around and push the entire podium on Officer Payne and stepped over the podium and began striking her with a closed hand ?st multiple times, immediately Of?cer Barbra Johnson sprayed him with one burst 0C spray it had no effect on him he turned away from Officer Payne and began striking Of?cer Johnson multiple times with a close hand fist. At that time 1? CS White administered one burst of OC Spray directly in the face of Offender Nelson/#67533 and that is when he charged at me and knock me down to the ground and he hit me (CS White) on the left side of my jaw at that: time 1? hit him with the OC Spray can and it fell out of my hand and he hit me again in the law with a closed hand fist 1? CS White hit him back with a closed hand fist and regain my OC Spray can back and administered another burst of QC Spray directly in Offender Nelson 67533 face and he ran on towards A~Zone and Of?cer Payne managed to get the radio and called for all available staff. At approximately 0824 hour?s all available staff entered Captain Mumford and Field Operation Tyler was the first to answer and when they entered A~Zone Offender Nelson it 67533 began to strike Captain Munford and Of?cer Tyler and he was also, shoving and pushing on both Captain Mumford and Officer Tyler that time Captain unford and Field Operation Tyler struck at Offender Nelson with the necessary amount of force to bring him into compliance. At approximately 0826 hour?s Warden Timothy Morris, Deputy Brenda Cox, Of?cer Rodgers, Devan Clark, Lt. Nathan Harris, Captain Mary Jones, Deputy Warden Simon, and Lt. Judy Anderson entered Unit 29 Building. At: approximately 0828 hour?s Captain Mumford and Captain Jones departed Unit 29 Building with Offender Nelson 67533 cnroutc to Unit 29 holding tank.
