3? RE: Hazel McGary Greetings, My name is Cheryl Singleton, an inmate at FCI Aliceville, contacting you regarding the recent death of the inmate listed above. I gathered information from her live-in roommate name Green. Below are the details of what lead up to the death. Inmate Hazel McGary was pronounced dead on 03/18/2019 around 12pm-ish. A. "Death" On 03/18/19 around 5:15am, Ms. Green found Hazel on the ?oor in their cell. Ms. Green stated to Hazel, "Why didn't you call my name like you usually do?" Hazel replied, tried to call you as loud as I can but you couldn't hear me." Ms. Green got Hazel off the ?oor and put a blanket on her. Then Green proceeded to the sink to run cold water on a towel for Hazel. Green placed the towel on her neck and tried to get Hazel to stand up properly. Green notice Hazel was weak and they both agreed to press the "medical/duress button" to alert prison of?cials. Within (5) minutes, medical staffs arrived. The medical staffs were: Lieutenant Cash, Lieutenant Bailey, and the of?cer on duty, Mr. Marshall. They took her to medical and that was the last time Green seen Hazel alive. Hazel falls frequently on the?oor from her bottom bunk bed. This occurs during the night and either Green or Hazel work among each other to see if its' serious or they press the medical/duress button to notify prison of?cials. B. "The Suspicious Pill" On 03/16/2019, two days prior to Hazel passing, she took a "suspicious pill" that was given to her by paramedic, Mr. Potter. This "suspicious pill" was a different pill that she usually takes. It made Hazel heart beat really fast and she was panting all day. This panting feeling continued until the last time I seen her. Hazel was panting so hard that she had to sit up at a 90 degree angle just to be able to breath.~ . C. "Trips to Sick Call/Medical" Green took Hazel to sick call/medical numerous times. A minimum?of three times a week or more. The majority of the sick call visits was the same procedures. Taking blood pressure, which was usually extremely high, and getting told from medical staff to come back later. When Hazel comes back later, as directed, she was told the issue is noted in her records and nothing else was done. During a trip to sick call/medical, Green observed medical assistant, Ms. Berry, falsify a blood pressure results from Hazel. Medical assistant, Berry, changed Hazel blood pressure from being abnormal high to regular. D. - "Fake Shot/Injection" Green and Hazel went to sick call to report her sickness which contains: fatigue, blurriness, fever, and etc. She was taken into a small medical room in the back. In the medical room, Mrs. Knopp, RN, allegedly stated that she gave Hazel an injection for pain the day prior. Knopp asked, "Are you feeling better and did it work?" After the question, Knopp burst out in laughter as if it was a joke and didn't even care if Hazel responded back or not. Medical?nor Mrs. Knopp, RN, never gave Hazel an injection for pain. Nor medical treated her chest pain. Medical took Hazel condition as a joke, which cause her to lose her life. This is clearly negligence. E. "Meeting with staff at Aliceville" Green was present at a meeting, 02/21/2019, that was conducted in the cafeteria with higher authorities of FCI Aliceville. These people were: Assistant Warden-Mr. Thomas, Assistant Warden- Mr. Massagdale, Medical Records- Mrs. Hester, Medical Director-Mrs. Eli, Knopp. Hazel shared her story. Hazel went into details of her medical condition and told them how it wasn't being treated. She explained to them how her health is deteriorating and she is in severe pain everyday. Hazel repeatively stated how can barely breathe, which lead her being con?ned in wheelchair. She told them how healthy she was before she got to FCI Aliceville and all of this started when she was in the Special Housing Unit. F. . ?Special Housing Unit" Hazel was in the "special housing unit" when all of this started to happen. The "special housing unit" is a 8'10 cell which you are con?ned to it for 23 hours a day. With this being her first time in the "special housing unit," she told of?cials about the panic attacks she was having, and nothing was done. She had many panic attacks, which caused her to breathe heavily aftenNards. She complained to the nurse on duty that she couldn't breathe and was having chest pain. Nothing happened as a result from her complaints. Hazel was an "camp (lower security level) inmate" and was housed by herself due to. the restrictions of her being housed with other inmates. Hazel was in the "special housing unit" for two week. Within these two weeks, Hazel feared being alone during these panic attacks, but of?cials didn't do anything about it. G. "Standing up for "4pm Count" Seems to be a Joke for Upper Prison Authority Of?cials" During the month of March, Hazel was barely standing up during a "4pm Count." During the count, Lieutenant St. John, laughed out loud in Hazel face stating, "you can stand up, what an miracle!" Lieutenant St. John thought "this barely standing" was so hilarious. Not only she thought it was hilarious, but she also blurted out "she thought it was a miracle." What upper authority prison of?cial would laugh at an inmate due to their health? Yes, it was a miracle to stand up for 5- minutes, due to Hazel poor non-medical treatment she was receiving at FCI Aliceville. Lieutenant St. John made a monarchy out of this and thought it was so hiliarious. Hazel health was in danger with upper authority laughing at her like its' a joke. H. ?Upper Tier, Second Floor with Climbing Stairs" Hazel health started to deteriorate since she got to FCI Aliceville in January 2019. From January 2019 to Mid-February, Hazel and Green lived on the upper tier/second ?oor, in Charlie Bravo Unit. The cell room number was 221. Green and Hazel lived there for 1.5 months. Hazel requested to be moved to the bottom tier several times due to her health, but all requests got ignored by of?cials. Attempts were made to counselor(s), case manager(s), unit team manager(s). All was informed, but no avail. With the help of other inmates, protesting on Hazel's medical condiction, she was eventually moved to the bottom tier/bottom ?oor. I. "No Wheelchair" Hazel was without a wheelchair for 1.5 months before receiving one. Hazel health was deterioraiting so bad that it made her very fatigue and unable to walk. Hazel noti?ed medical that she needed a wheelchair. When Hazel was on the upper tier/?oor, she needed a wheelchair. Prior to Hazel receiving a wheelchair, Green would assist Hazel to walk up the ?ight of stairs, and to other places, such as the shower. Hazel had a hard-time getting from one place to another and always needed assistance. Due to not having a wheelchair for an 1.5 months, Hazel couldn't eat in the cafeteria for breakfast, lunch, nor dinner. Hazel condition was severe, which caused her not to be able to walk. Prison of?cials lacked attention and medical care for her. Hazel got a wheelchair the ?rst week of February. J. . "Pressing the Medical Alert Button" In mid January around .11am, Hazel pressed the "medical button" that is located in the cell. This button is ?only for medical/duress emergency only. When prison of?cials arrived to the cell, which were: Lieutenant Wilcox, Lieutenant Redd, and Mr. Lockett, they all asked "what is your medical emergency?" Hazel stated that she is having chest pain and need to be seen by medical. They all replied "medical is refusing to see me." Hazel then asked one more time, "ls'medical refusing to see me, or are they here?" The answer was still the same, "Medical is refusing to'see you." Then they all replied, "Do you want to go to the "Special Housing Unit" to be monitored every ?ve minutes?" Hazel replied, "No" and they all left her cell. K. Trip to Medical" One morning in January on a rainy day, Hazel and Green went to sick call due to chest pain. On the way there, it took Hazel approximate 20 minutes to walk to medical from her housing unit. At this time, Hazel didn't have a wheelchair. Mrs. Knopp, RN, seen Hazel struggling to get to medical while she was standing at the door. Nurse Knopp didn't bother to ask her for assistance. Hazel almost collapse while she was struggling and medical staff didn?t care. .L- Mystery" During a visit to sick call, medical decided to perform an EKG on Hazel. It was conducted in front of Green. During the EKG, Green witness multiple EKG's that was completed, re-done, printed, torn up during the one-time session. Hazel believe that is was done multiple times due to the results of the EKG. At least 6 results that was printed was ripped up, and the last one was kept for their records. Did medical tried to hide Hazel abnormal Or did medical perform multi to see which one looks normal on paper? "Hazel Begged for her Medicine" In February 2019, Hazel went to "sick call/medical" toget her medicine. Hazel ran out of medicine and needed more. The normal procedure of receiving medicine at Aliceville is to logon into the portal and press "re?ll." It takes approximate 5-7 days before your medicine arrives. It was passed 5-7 days and Hazel went to "sick call/medical" to pick up her medicine. She asked the paramedic, Mr. Potter, for her medicine. He refused to give it to her. She ask him one more time for it, and he threaten to send her to the Special Housing Unit. Hazel then stated, "Mr. Potter, all i want is my medicine." Mr. Potter than grabbed his communication device, "walkie talkie" and told prison of?cials that "an inmate is refusing to leave medical." Prison of?cials arrived and escorted Hazel off from medical. The of?cer that arrived to escort Hazel knew Mr. Potter was wrong and said a prayer for Hazel while walking her back to the housing unit. - All Hazel wanted was her medicine for her chest pain. She came to medical to decrease her pain by her medicine, but instead Hazel pain was increase due to Mr. Potter behavior. Why do any human being have to begged for medication that will protect their health? This is inhumane and FCI Aliceville needs to be accountable for their actions. M. "Regional Director of Medical, Mrs. Hunterbuskin" In February, Hazel went to "sick call/medical" and to her surprise, the Regional Director of Medical from Federal Bureau of Prisons was there, Mrs. Hunterbuskin. Hazel was very excited to ?nally tell someone with upper authority about medical and her health. Hazel talked to Mrs. Hunterbuskin about her health, medical, her treatment at Aliceville from medical, and her heart problems. Mrs. Hunterbuskin responded abruptly with a question of "how old are you?" Hazel responded with her age and then she was interrupted by Hunter buskin stating, "there is no way you can have heart problems at your age, unless you are a dope ~addict or have an severe infection." Hazel then replied, "my chest hurts and I know my heart is failing." Mrs. Hunterbuskin stated, "you need to get your head examine to see if you are crazy." Hazel couldn't believe she was hearing this from the regional director. To hear this remark from an upper authority, Hazel knew what she was facing. An prison facility who don't care about inmate's health deteriorate nor their well-being. Hazel still told Mrs. Hunterbuskin that she was in pain and she died to this pain. Sick Call/Medical- a place where inmates seeks medical emergencies/issues Upper Prison Of?cials- higher authority among FBOP system Bottom tier- ?rst floor . Upper tier- second ?oor FBOP- Federal Bureau of Prisons 4pm Count- a head count on inmates to calculate the number of inmates.
