Bolton-St. Johns, LLC agrees to provide Greater New York Hospital Association, Inc. for the
bene?t of Greater New York Hospital Association, Inc. with legislative and regulatory
representation in New York State and the City of New York from January 1, 2020 through
December 31, 2020. The fee shall be payable beginning January 1, 2020 as follows:

0 $14,167 per month for NYS lobbying and $10,000 per month for NYC lobbying

After the initial ninety (90) day period, this agreement is cancelable upon thirty (30) days written
notice. Other services in addition to lobbying will not be provided by the Lobbyist.

If the above meets with your approval, please so indicate by signing and dating this letter in the space
provided below and e-mailing a copy to our Albany of?ce. We will ?le a copy of this signed letter
with the New York State Joint Commission on Public Ethics and the Office of the City Clerk of
the City of New York-Lobbying Bureau.
