Headley, your waiver of a due process disciplinary hearing and your overall work record, you are hereby suspended without pay for a period of two (2) days to be effective October 24, 2016 through October 25, 2016 inclusive. On August 4, 2016, you were assigned as the Third (3m) Shift Commander at Elmore Correctional Facility. At approximately 10:00 you relieved the Second Shift Commander who reperted to you that there were inmates being held in the Transfer Cells at the Back Gate. You instructed a Correctional Of?cer to assume duty at the Back Gate. You have been instructed by the Captain not to house inmates in the Transfer Cells at the Back Gate on Third Shift. You failed to follow instructions from the Captain by assigning an Of?cer to the Back Gate to monitor three (3) inmates that were housed there. During an institutional recount, you entered the Back Gate after being informed by the Officer that an inmate was missing. You then discovered that an inmate had escaped. Your actions constitute a violation of the following standards under Administrative Regulation 208, Employees Standards of Conduct and Discipline: 1. Employees shall render full, efficient, and industrious service. (Section V, Paragraph A2) 2. Employees shall reapond to directions and instructions of supervisor. (Section V, Paragraph A3) 3. Employees shall observe all laws, rules and regulations. (Section V, Paragraph A7) 4. Employees shall uphold, with integrity, the public?s trust involved in their position. (Section V, Paragraph A8) 5. Each employee?s conduct shall, at all times, be consistent with the maintenance of proper security and welfare of the institution and of the inmates under his/her supervision. Failure to follow supervisor?s instructions; non-compliance with policies and procedures. (Administrative Regulation 208, Annex 1-1, Number 1 1) A review of your overall work record reveals the following active corrective actions: 1. Pending: Suspension Failure to performjob properly (7/29/ 16). 2. Pending: Written Reprimand Failure to follow supervisor?s instructions; non- compliance with policies and procedures (5/20/16). 3. January 16, 2016: Written Reprimand Failure to performjob properly, not resulting in actual consequences (10/28/15). 4. January 16, 2016: Written Reprirnand Failure to perfonnjob properly, not resulting in actual consequences (9/1 5). The intent of this action is to emphasize the necessity for you to follow rules and regulations. Any similar infractions after this incident may result in further corrective action.
