On October 9, 2010, you were scheduled to report to work at 6: 00 a. m. at Elmore Correctional Facility but you failed to report to work as scheduled At approximately 6 .30 a. no. ,you reported to work. Your actions constitute a violation of the following standards under Administrative Regulation 208, Employee Standards of Conduct and Discipline: 1. Employees shall report for work on time and in a condition to perform their job properly. (Section V, Paragraph A1) 2. Employees shall render full, efficient, and industrious service. (Section V, Paragraph A2) 3. Employees shall observe all laws, rules and regulations. (Section V, Paragraph A7) 4. (Administrative Regulation 208, Annex H, Number 7) A review of your overall work record reveals the following active corrective actions: 1. December 7, 2010: Suspension Unexcused absence. 2. October 13, 2010: Written Reprimand Late for work. 3. September 2, 2010: Warning Late for work. The intent of this action is to emphasize the necessity for you to follow rules and regulations.
