Waver, Correctional Sergeant Elmore Correctional Facility Elmore, AL 36025 Dear Mr. Waver: After having reviewed the recommendation of Warden Leeposey Daniels, your waiver of a due process disciplinary hearing and your overall work record, you are hereby suspended without pay for a period of two (2) days to be effective December 7,2010 through December inclusive On August 3, 2010, you were scheduled to report to work at 6: 00 am at Elmore Correctional Facility but you failed to report to work as scheduled. At approximately 6: 15 a. the Lieutenant attempted to contact you, via telephone, but was unsuccessful On July 31, 2010, you submitted a leave request in Kronos but that request was denied. Your actions constitute a violation of the following standards under Administrative Regulation 208, Employee Standards of Conduct and Discipline: 1. Employees shall report for work on time and 111 a condition to perform their job properly. (Section V, Paragraph Al) 2. Employees shall render full, efficient, and industrious service. (Section V, Paragraph A2) 3. Employees shall observe all laws, rules and regulations. (Section V, Paragraph A7) 4. Employees shall uphold, with integrity, the public? trust involved in their position. (Section V, Paragraph A8) 5. Employees shall obtain approval for any absence from work. Tardiness, failure to follow proper call-in procedures, and unexcused absences shall subject the employee to disciplinary action. (Section V, Paragraph A18) WaverKD Page 2 Kenny D. Waver In determining the appropriate corrective action for the above violations, I have considered the following infraction: Failure to report to work (unexcused absence). (Adn?nistrative Regulation 208, Annex H, Number 27) A review of your overall wOrk record reveals the following active corrective action: August 11, 2009: Suspension Abusive or excessive physical force. The intent of this action is to emphasize the necessity for you to follow rules and regulations. Any similar in?ections after this incident may result 111 further corrective action.
