Dear Mr. Waver:

On September 8, 2016, a hearing was held based on Warden Willie Thomas? recommendation that you be
suspended without pay for a period of two (2) days. The Hearing Of?cer found you guilty of all charges
against you under Administrative Regulation 208, Employees Standards of Conduct and Discipline:

1. Employees shall render full, ef?cient, and industrious service. (Section V, Paragraph A2)

2. Employees shall reapond to directions and instructions of supervisor. (Section V,
Paragraph A3)

3. Employees shall observe all laws, rules and regulations. (Section V, Paragraph A7)

4. Employees shall complete a written report (ADOC Form 302-A, Incident Report) of all unusual
incidents that occur during a tour of duty. (Section V, Paragraph A15)

In determining the appropriate corrective action for the above violations, I have considered the following
infractions:

1. Failure to perform job preperly, not resulting in actual consequences. (Administrative Regulation
208, Annex H, Number 10)

2. Failure to follow supervisor?s instructions; non-compliance with policies and procedures.
(Administrative Regulation 208, Annex H, Number 11))

On November 15, 2015, an inmate was escorted to the Health Care Unit for an unknown illness by the
Sergeant at Bibb Correctional Facility. You failed to follow up on this incident and ensure that an incident
report and body chart was completed in a timely manner.

A review of year overall work record reveals the following active disciplinary action:

1. June 3, 2015: Written Reprimand Fail to perform job properly, not resulting in
actual consequences

Having reviewed the record of hearing and your overall work record, I do hereby approve the Hearing

Of?cer's ?nding. Therefore, I am ordering your suspension without pay for a period of two (2) days to be
effective December 13, 2016 through December 14, 2016 inclusive.

The intent of this action is to emphasize the necessity for you to follow rules and regulations. Any similar
infractions a?er this incident will result in ?rrther corrective action. Hopefully, your actions in the future
will meet standards.
