Overall Inspection Comments
B.Whitney conducted a complaint investigation regarding facility that was observed with a insects and mouse problems. At time
of complaint investigation B.Whitney observed approximately two dead and squished roaches at bar area. Inspector observed
two roaches on their back with legs moving by door adjacent to kitchen. Observed three traps full of dead roaches throughout
kitchen and fresh mouse dropping below stove and clean utensils and equipment. Observed pan on floor below grill with dead
roach. George, person in charge stated facility is serviced once a month and was last serviced on Monday 6/9/2020 (service
record provided). George, person in charge has agreed to voluntarily closed at this time due to vermin activity. Facility has been
instructed to properly clean and sanitize the affected areas and provide proper pest control and exclusion of facility. Facility is to
be free of vermin and verified by a member of this Department prior reopening. Closure sign posted and "A" placard removed
at this time. Closure sign must remain posted until removed by an employee of this Department. If you have any questions
