Overall Inspection Comments
This was a reinspection from earlier foodborne illness investigation. At this time inspector observed numerous flies in food
preparation area and contamination of in use open flour mixture used for breading chicken, utensils, and other food contact
surfaces (food preparation tables). Facility had pest control company come before reinspection and they repaired the back fly
light and sprayed around outside dumpster area. The manager on duty was not able to protect food and food contact surfaces
from being contaminated and did not voluntarily close. Facility has been instructed to discard all contaminated food and to
properly clean and sanitize all affected food contact surfaces. Inspector observed employees discarding all affected food at this
time.
Facility permit has been temporarily suspended, immediate closure paperwork delivered due to immediate health hazard and
facility is to remain closed until fly infestation has been abated, affected food contact surfaces properly cleaned and disinfected,
hearing is held, and permission is granted by this Department. A hearing has been scheduled for 6/25/2020 at 9:00 am and will
be conducted virtually (link to hearing will be provided in email to facility). Facility closed sign posted, "A" placard removed. Do
