Dannonhower reported the suicide death of a 20 year old male, later identified as Christian Madrigal, who died in the Intensive Care Unit (ICU) of Eden Medical Center in Castro Valley. According to Nurse J. Dannonhower, on Monday, June 10, 2019, Madrigal hanged himself while at Santa Rita Jail (SRJ) in Dublin. Madrigal was administered Cardio Pulmonary Resuscitation (CPR) at the jail by first responders who transported him to Stanford ValleyCare Hospital in Pleasanton. Madrigal was later transferred to Eden Medical Center in Castro Valley the same day for a higher level of care. Madrigal was placed in the Intensive Care Unit (ICU) of Eden Medical Center where his health continued to decline until he was pronounced brain dead. On Friday, June 14, 2019, at 1430 hours, Dr. D. Bonovich pronounced Madrigal's ?rst brain death. On Saturday, June 15, 2019, at 1052 hours, Dr. C. Walker pronounced Madrigal's second brain death. (JSH1432) Related Medical Information: According to medical records, Madrigal had ligature indenture marks on the front and sides of his neck, which were reportedly from steel leg iron restraints. Digital photographs were requested of the leg irons that were placed into evidence, locker #77, at Santa Rita Jail. The digital photographs were provided and I later downloaded them onto a computer disk and placed them in Madrigal's case ?le for future reference. On Saturday, June 15, 2019, about 1304 hours, I (Hovda) received an email from Jaclyn Manzanedo at Donor Network West that contained Madrigal's medical records from Stanford ValleyCare and Eden Medical Center. I forwarded Madrigal's medical records to Coroner's Pathologist Dr. A. Chen" for her review. Donor Network West identi?ed Madrigal as a ?rst person donor via his California Driver License application. I spoke to Dr. A. Chen who ultimately determined Madrigal's organ donation could proceed with the following restrictions in place; Madrigal was not to have any organs removed without Dr. A. Chen being present and the areas of the neck, head, spleen and liver, were not to be harvested. Lt. D. Vandagriff also placed the additional restriction that an Eden Township Substation Investigations Detective be present during any organ donation procurement. Jaclyn Manzanedo was informed, via email, and agreed to the organ donation restrictions. On Saturday, June 15, about 1930 hours, I (Hovda) spoke with Jose Jaime at Eden Medical Center in Castro Valley. Jaime stated he and his wife, Gabriela, were opposed to any organ donations. I explained to Jaime that it was up to the Coroner's Pathologist to determine if an organ donation would interfere with pathological findings that would determine the cause of Madrigal's death. I also explained that it was Madrigal's own personal consent and desire to grant his organ donation when he applied for his California Driver's License, which superseded any of the family objections. On Saturday, June 15, 2019, at 2030 hours, I (Hovda) recovered three vials of Madrigal's admittance blood from Eden Medical Center. I later inventoried the vials of blood and placed them into the main Coroner's Bureau refrigerated morgue for oathological review. In addition, I (Hovda) compared the decedent to the photograph image and general description on California Driver License belonging to Christian Eduardo Madrigal/Covarrubias and they appeared to match. On Saturday, June 15, 2019, about 2100 hours, I (Hovda) called and spoke to Madrigal's step father, Jose Jaime, who stated the decedent should be referred to as Christian Eduardo Madrigal on his death certi?cate. (JSH1432) On Wednesday, June 19, 2019, about 1051 hours, I (Wilson) received a fax from Alameda County Central Identification Bureau (CIB). A comparison was made between the decedent's fingerprints and the fingerprints associated with Cll# 38549311 with the name: Christian Covarrubias Madrigal with a DOB: 8/30/1998. The ?ngerprints were identi?ed to have been made by the same subject. (BAW1497) Last Known To Be Alive: . Madrigal was last known to be alive on Saturday, June 15, 2019, at 1052 hours, when his second brain death was pronounced at Eden Medical Center in Castro Valley. (JSH 1432) Next Of Kin investigation: According to Madrigal's step father, Jose Jaime, Madrigal was not married and had no children. Madrigal's mother, Gabriela Cobvarrubias, is his legal next of kin. Cobvarrubias was notified by hospital staff of Madrigal's death on Saturday, June 15, 2019, prior to my arrival at Eden Medical Center. On Saturday, June 15, 2019, about 1930 hours, i (Hovda) arrived at Eden Medical Center in Castro Valley to assist Coroner's Pathologist Dr. A. Chen with taking digital photographs of Madrigal prior to his organ donation procurement. Gabriela and Jose Jaime were both present in the Madrigal's ICU. room. Gabriela appeared visiblytupset and left the room. I spoke to Jose Jaime who was present in Madrigal's I.C.U. room and he agreed to relay my information to Gabriela. explained the Coroner's Bureau involvement in Madrigal's death investigation and advised him to assist Gabriela in making funeral arrangements as soon as possible. Holt and confirmed the press hold was still requested to remain in place. Per Detective L. Holt, the press hold shall remain in place until the Coroner's Bureau autopsy protocol, written reports and all investigation were completed. (JSH 1432) Property And Evidence: On Saturday, June 15, 2019, about 2030 hours, i (Hovda) issued Coroner's Bureau receipt #40506 to theblood lab technician for Madrigal's admittance blood that recovered from Eden Medical Center. Nothing else was inventoried on the receipt at that time. According to Alameda County Sheriff's Office report #19-009845, several items were recovered and processed into evidence and placed into evidence, locker #77, at Santa Rita Jail. Items collected included "Jordan" brand white sneakers (size 9.5), one white sweater brand, "Aeropostale" blue jeans, one red rubber bracelet, one spit mask, a pair of grey "Peerless" handcuffs, "Smith and Wesson" leg irons, a grey ACFD tarp, (2) white cleaning cloths, crumpled toilet paper, a medical AED card, and an opened juice packet. (JSH1432) Coroner Fees: As of June 15, 2019, there were Coroner's Bureau fees for body preparation in the amount of $67. There were no removal fees as Donor Network delivered Madrigal's body to the Coroner's Bureau. As of Friday, June 21, 2019, all of Madrigal's Coroner's Bureau fees had been paid in -full. (JSH1432) Case Narrative: On Saturday, June 15, 2019, about 1730 hours, (Hovda) called and spoke to Jose Jaime, Madrigal's step-father, to explain the Coroner's Bureau involvement in Madrigal's death investigation and to make sure the family was notified of his death. Gabriela Cobvarrubias, who is the legal next of kin in this case, wanted her husband Jaime to be the point of contact with the Coroner's Bureau on her behalf. Jaime told me the following in summary concerning the events that occurred prior to Madrigal's arrest. Jaime had initially called the Fremont Police Department (FPD) on Sunday, June 9, 2019, because Madrigal was acting aggressively and irrationally after consuming psilocybin mushrooms. Jaime said he knew Madrigal was on these drugs because Madrigal verbally admitted to him that he had consumed the mushrooms. Jaime stated when FPD officers ?rst arrived at their residence, at 2100 hours, they did not believe Madrigal met the criteria for a 5150 W&l Mental Health Evaluation Holdand they left despite his pleas to place him on a hold. Madrigal 's bizarre and aggressive behavior continued throughout the night. The following day, on Monday, June 10, about 0600 hours, Jaime again called the Fremont Police Department, a second time, because he was still concerned about Madrigal's mental state and continued aggressive behavior. The FPD Of?cer who arrived at the residence stated he also felt Madrigal did not meet the criteria for a 5150 W&l hold. However, the officer placed Madrigal under arrest for being under the in?uence of a controlled substance and transported him to the Fremont City Jail. While in the custody of FPD, (it was noted in their police report) Madrigal was continually resistive and uncooperative with police orders and had to be placed in a "Wrap" restraint device to overcome his 'esistance. Madrigal was later transported to the Santa Rita Jail in Dublin. 3n Saturday, June 15, 2019, about 1800 hours, I was forwarded the ACSO body worn camera footage that captured l/ladrigal's movements while being booked at Santa Rita Jail. I reviewed the videos provided and observed the following in ;ummary; Madrigal arrived at SRJ still in a "Wrap" restraint device. He was cleared for incarceration by nursing staff and :arried to cell R-1 while still being restrained in the "Wrap". The restraints appeared to be all connected together in chain. One set of handcuffs was secured to the cell's exterior door knob and locked together at one end of the leg iron ankle cuff. The leg iron ankle chain was then passed through the open cuffing port of the cell door into the cell and attached to the other set of handcuffs that had been placed on Madrigal's wrists, behind his back. The "Wrap" was then removed from Madrigal and the cell door was closed, with Madrigal restrained to the door knob and being the sole occupant inside the cell. i (Hovda) viewed subsequent body worn camera footage that showed deputies returning to Madrigal's cell where he appeared to be unresponsive. At that time, deputies are seen opening the cell door and saw Madrigal had been able to "slip" the handcuffs from behind his back to his front. Madrigal was seen in a seated position with the leg iron chain wrapped around his neck. Deputies placed Madrigal on the floor and began C.P.R. until they were relieved by SRJ nursing staff. Nursing staff was then relieved by responding Paramedics and Madrigal was ultimately transported from the scene to ValleyCare Medical Center in Pleasanton. On Saturday, June 15, 2019, about 1930 hours, Corner's Pathologist Dr. A. Chen and (Hovda) arrived at Eden Medical . Center in Castro Valley to take digital photographs of Madrigal prior to any organ donation procurement taking place. Madrigal was located in the intensive Care Unit I identified Madrigal by his hospital wristband and by comparing him to the photograph image and general description on his driver's license. Jaime was also present in the when Dr. A Chen and arrived. i asked Jaime to leave the room so that Dr. A. Chen and i could conduct an examination of Madrigal's person. During the exam, i saw there were ligature indenture marks around Madrigal's neck and other various scrapes and bruises present on his body. The scrapes and bruises did not appear to have any contributory factor in his death as most appeared to be old and in the healing process. assisted Dr. A. Chen in taking digital photographs of Madrigal. These photographs were later downloaded onto a computer disk and placed in Madrigal's case file for future reference. On Saturday, June 15, 2019, about 2030 hours, i (Hovda) recovered three vials of Madrigal's admittance blood from the Eden Medical Center blood lab. On Saturday, June 15, 2019, about 2045 hours, Coroner's Pathologist Dr. A. Chen and i (Hovda) returned to the Coroner's Bureau. labeled and inventoried Madrigal's admittance blood prior to placing it in the main cooler of the morgue for future pathological review. On Monday, June 17, 2019, Coroner's Pathologist Dr. A. Chen was summoned by Donor Network West to attend Madrigal's organ procurement/donation. Prior to the procedure, Dr. A. Chen stated she examined and photographed Madrigal's liver and did not observe any injuries to the liver that would have caused Madrigal's death. Chen authorized Madrigal's liverto be donated?. (JSH1432) On Tuesday, June 18, 2019, around 0715 hours, an agent of the Donor Network West delivered Madrigal to the Coroner's Bureau. Madrigal was nude in a white body bag, face up on a gurney and medical therapy still left in place. I (Wilson) saw a dark abrasion around his neck. Madrigal had an incision down the center of his chest and abdomen due to the organ donation. processed Madrigal into the morgue, took intake photos of him and placed him into refrigeration. (DLW29) On Tuesday, June 18, 2019, Coroner's Pathologist Dr. A. Chen performed a full autopsy in order to determine Madrigal's 3ause of death. Dr. A Chen gave a cause of Madrigal's death of "Anoxic encephalopathy" due to "Hanging". 3n Friday, June 21, 2019, about 1540 hours, Madrigal was released to J. On Friday, September, 20, 2019, about 1030 hours, I (Hovda) reviewed Madrigal's case for closure and determined manner of death to be a suicide. My findings were based on my investigation which included; the cause of death given by Coroner's Pathologist Dr. A. Chen, the videos depicting Madrigal as the sole occupant of a jail cell with a chain ligature around his neck, Alameda County Sheriff's Office reports, and statements made by Madrigal's step father concerning hrs'mental health prior to death. Madrigal had no other contributory injuries on his person, excluding the ligature marks on his neck, therefore it did not appear he was the victim of an assault. His initial police contacts were a result of his own family's concerns for his mental health and safety. Madrigal's step father told me he called the Fremont Police Department because the family felt unsafe around Madrigal due to his unstable behavior and paranoia. It appeared Madrigal was able to place himself In a seated position and hanged himself after he was able to move his handcuffed hands from behind his back to the front, and then used the leg iron chain as a ligature. Therefore; I believe Madrigal intentionally caused his own death by hanging himself. (JSH1432) Supervisory Review: On Friday, September 20, 2019, (Sgt.
