As a group, strategic decisions were made by the ELC Governance area leads in consultation with the principal investigator, Dr. Sarah Park, and the Director of Health, Dr. Bruce 8. Anderson, to enhance detection of in the State of Hawaii by providing critical infrastructure resources to support testing and epidemiologic surveillance and investigation related activities. The team plans to leverage and build upon already existing ELC infrastructure and further coordinate and integrate epidemiology, laboratory, and health information systems in public health to achieve our plan objectives. In addition, the governance team will ensure Hawaii has the leadership staff to manage this award. Previous and current ELC funds have ensured Hawaii can improve our infectious disease prevention programs; build robust health information systems; and provide adequate staffing of appropriately trained personnel, including epidemiologists, laboratory scientists, and information technology specialists to address infectious diseases, especially as we respond to the pandemic. Hawaii?s ELC project plans to hire and train staff to increase capacity for data management, analysis, and reporting of We will strengthen and expand laboratory testing for as well as expand community-based surveillance to scale testing to the assessed need and demand. This expansion in testing will require enhancements to data management and analytic capacity in our public health laboratories to optimize operations, management, and testing as well as data sharing. Further, these efforts will advance electronic data exchange by improving surveillance and reporting of electronic health data, with an emphasis on automation and a consideration of the resources required to accomplish it. By so augmenting ELC Governance Team Letter May 23, 2020 Page 2 our overall data management, we will increase our capacity to conduct investigations, identify those cases in high-risk settings, and implement prevention strategies. Lastly, the Hawaii Department of Health plans to boost capacity for infection prevention and control in our long- term care facilities and other congregate settings. Funding from CDC's Crisis COVID-19 Notice of Funding Opportunity (NOFO) allowed Hawaii to quickly respond in the early days of the pandemic. Funds provided critical support to ensure our immediate response as well as lay the groundwork to prepare for the coming activities. Specifically, these funds allowed us to jump start our public health laboratory testing capacity, ensure wraparound services for persons in isolation and quarantine, establish a call center to manage the onslaught of calls, and initiate laboratory information systems support among other capabilities. CARES funding allowed Hawaii to begin the investment to support both the immediate investigation/contact tracing, data management, and laboratory capacity needs but also begin laying the groundwork in these areas for our long-term response. With ELC Enhancing Detection (ELC-ED) funds, we can further expand and even build our ideal laboratory testing capacity, informatics workforce and systems, and infection control and prevention team. Additionally, we can ensure a comprehensive applied epidemiology team at every level of needed expertise, from contact tracers to investigators to epidemiologists, and across the state in all Counties to enhance data collection, analysis, reporting, and visualization. In the area of laboratory testing, we can truly become a state of the art reference facility with high throughput molecular and serological testing as well as whole genome sequencing and analysis capacity. These capabilities, in the setting of ELC-ED funded improvements to the State Laboratories' infrastructure to ensure laboratory worker safety, will greatly supplement the work of the applied epidemiology team to conduct timely investigations, surveillance especially of at-risk or congregate populations (e.g., underserved cultural communities, university settings, air travelers), and analyses to inform prevention measures and recommendations as well as policy decisions. With regard to data management and reporting systems, Hawaii will gain much needed capacity to finally establish syndromic surveillance to collect admissions symptom data as well as electronic case reporting from hospitals. Existing systems (i.e., electronic laboratory reporting, disease case management, and laboratory information systems) will all get a much needed boost. Finally we will have the robust healthcare associated infections team to provide regular and immediate technical support for all types of healthcare facilities (i.e., acute care, critical access, long-term care, skilled nursing, dialysis centers, etc.) to rapidly mitigate and prevent crises from potential COVID introductions. 06/18/2020 6:17pm projectredcap.org Confidential Recipient: HI Page 2 2. COVID-19 case data have been presented publicly since the beginning of this pandemic on our COVID-19 website: https://health.hawaii.gov/coronavirusdisease2019/what-you-should-know/current-situation-in-hawaii/. As of June 15, 2020, data have been presented in a dashboard format using ArcGIS and Tableau platforms to provide both data visualizations and tables. Additionally, surveillance data (i.e., ILI/CLI sentinel surveillance data) are reported weekly (https://health.hawaii.gov/coronavirusdisease2019/for-clinicians/sentinel-and-influenza-surveillance/) with the intent to add sections to this report as systems are developed and as relevant (e.g., air traveler surveillance). 4. Please describe how local needs will be assessed, prioritized, and incorporated into the priorities and funding decisions being addressed at your health department, in accordance to the activities outlined in the guidance. Gaps have long been recognized across the state in workforce capacity for applied epidemiology, community outreach (especially to under served Pacific Islander groups), public health education, laboratory support, and informatics. All areas will be addressed as noted above in response to #1 to ensure a robust COVID-19 response in all rural areas (i.e., Kauai, Maui, Hawaii Island) of the state in addition to the one urban area (i.e., Honolulu). 5. Please describe the expected mechanisms and frequency of interactions between the health department and/or public health laboratory with academic/hospital and commercial laboratories. Please note any agreements between PHL and other testing entities that are currently in place for testing services as well as details regarding any plans to formalize such agreements in the future. Hawaii is a small state with a population of ~1.4 million and is geographically the most isolated land mass. The benefit of this is that the clinical laboratories here are few and captive. While early in the pandemic, the local laboratories had to partner with and use mainland laboratories to provide support for testing demand. Combined, they are now fully capable of addressing all Hawaii's testing needs. There are two major clinical commercial laboratories, one managed care laboratory, and one military-based laboratory; all located in Honolulu. The four comprise the majority of the testing capacity along with the State Laboratories Division (SLD). Two other small laboratories (one community hospital-based in Honolulu, and one stand alone on Hawaii Island) have developed limited COVID-19 testing capacity. The SLD Director has frequent and regular communications with all Hawaii's laboratories and surveys them ~every 2 weeks to determine testing capacity and issues. All our laboratories enjoy a collegial relationship through which they offer support to each other in the form of sharing supplies as needed during this pandemic. SLD performs validation for all newly established tests in the clinical laboratories as well as confirmatory testing for inconclusive results. Hawaii is fortunate to have such a robust, collaborative laboratory network that can meet the needs of our state. Implementation Plan Per CDC guidance, each LTC facility should have 1 full time IP per 100 beds. In our ICAR project (so far have data on 40 out of 48 SNFs) there are 4,033 SNF beds in Hawaii (this is an underestimation as there are 8 facilities not included in this data). So, need at least 40+ full time IPs in the SNFs. There are 17 facilities, of the 40 facilities for which we have data so far, which have over 100 beds which mean they should have more than one full time IP in the facility. Per CMS rule 483.80, all Long Term Care Facilities should already have one designated IP who is responsible for the facility's IPC program. Per this CMS rule, this person must have completed specialized training in infection prevention and control and works at least part-time at the facility. HDOH's approach is to build extensive IPC expertise within HDOH in order to leverage existing relationships with designated facility-based IPs, and close any gaps that may exist. This team of infection preventionists will support infection prevention education and outreach not just in long term care but across the healthcare continuum, including dialysis, dentist offices, ambulatory surgery centers, and more. HDOH-based IPC infrastructure will be critical to support all care settings as healthcare offices begin opening back up and require guidance on PPE use, appropriate testing strategies, and more. An expanded team of HAI-focused epidemiologists is also necessary to respond to single cases or clusters in any healthcare setting to ensure transmission is prevented or stopped. Due to the nature of outbreak response, which requires 24/7 on-call support to affected facilities, large amounts of data and information sharing, the setting of a case definition, creation and update of epidemiological curves and other data visualization and analysis, three more HAI epidemiologists are requested to supplement the two existing Epidemiology positions funded through ELC (original ELC). Implementation Plan To accomplish all that is required and planned to achieve the expected metrics, many more epidemiologists in specific subject areas/expertise (e.g. healthcare associated infections, syndromic surveillance, non-flu respiratory disease, big data, etc.) as well as cross-cutting positions must be brought on board. Additionally, an epidemiological specialist is needed to further increase our overall investigator capacity and therefore data collection via investigations and contact tracing. Having sufficient investigators will mean our epidemiologists can focus on organizing, analyzing, and presenting the data to meet demands for assessment, planning, and communications purposes. While the initial plan to meet the investigation and contact tracing demands of spikes in disease activity is to surge with staff from within the Department of Health, if there is a large and rapid rise in the number of cases, additional staff external to the Department may be required. Training and orienting such staff on contact tracing methods and our specific investigative protocols as well as vetting volunteers to ensure they appropriately meet our needs takes time and effectively places staff offline at at time when they are needed most. DOCD has collaborated with the University of Hawaii to establish a just-in-time, 1.5 day training program for those with appropriate clinical backgrounds as well as a longer, credited 6-week training program for those without a clinical background. The goal is to train over 300 person by the end of July 2020, which is anticipated to more than meet our needs should we require them. All those who are trained, will be registered with the Medical Reserve Corps so we may keep track of these certified individuals and hire them for 3 months at a time if needed. The Counties of Kauai, Maui, and Hawaii are rural areas with many access and capacity challenges. Additional staff are needed to support disease investigations and contact tracing as well as public health education and outreach activities, especially to disadvantaged cultural populations (e.g., Pacific Islander). Because of Hawaii's relative geographic isolation and small population, recruiting and hiring appropriately trained and experienced applied epidemiologists to fill positions is always challenging. Often we have to rely on persons from the mainland and hope they find moving to Hawaii attractive enough to offset the distance from family and the high cost of living. Similar to Florida's Field Epidemiology Training Program, we plan to establish a training program for at most one fellow every 2 years, with preference for a local resident and anticipation of hiring that person upon completion. Applied epidemiology relies on data for all our work. Many different surveillance systems and databases are now required to be established with ongoing maintenance and enhancement requirements. A full program, or branch, of staff are required to ensure all these systems are established and/or enhanced as well as to centralize data management and increase problem solving capabilities. We will establish a new program under DOCD to house all the information technicians, data scientists, business analysts, etc. to support the systems required for the various required electronic data exchanges and reporting. The COVID-19 Community Surveillance and Outbreak Response Unit will include an Epidemiologist II and 4 Epidemiologist I positions. Implementation Plan With expanded COVID-19 electronic laboratory reporting (ELR) requirements, including additional data elements to be transmitted in HL7 format, changes must be made to the underlying structure of the Electronic Communicable Disease Reporting System (ECDRS) data warehouse that processes ELR signals from clinical laboratories, as well as in our Maven-based Hawaii National Electronic Surveillance System (HI-NEDSS). Our current ECDRS was based on the now outdated National Electronic Telecommunications System for Surveillance (NETSS), and is in need of a complete overhaul and redesign to meet current needs, including ability to provide line-level ELR segments rather than aggregate counts of laboratory reports. A redesigned ECDRS will allow for accurate comparison of data obtained from ELR and message mapping guide (MMG) reports which are generated from HI-NEDSS, and ultimately result in higher quality of ELR data reported to CDC. The new ELR database will also greatly support other program partners (HIV, TB) that currently do not have the ability to import ELR into their disease reporting database systems, by allowing for integration of HL7 formatted ELR data segments with their respective systems. COVID-19 has created demand for multiple new and updated data systems and structures. A new database will be established to accommodate records that will be generated from Hawaii's newly established Traveler Health Screening program, which is anticipated to house data from thousands of arriving travelers each day. Multiple critical Maven updates are also needed to address the high volume of COVID-19 reports for both positive and negative results for PCR, antibody, and antigen-based testing. Additional staffing is needed to process the plethora of new laboratories and test types that must be added to ELR feeds on what has become nearly a daily basis. In the meantime, progress must continue to be made on establishing electronic case reporting (eCR) as well as syndromic surveillance in Hawaii. Hawaii is currently working with a pilot site to begin receiving eCR and SyS test messages, with support from the Epidemiology and Laboratory Capacity cooperative agreement, but additional staff will be needed to meet the new demands of COVID-19 reporting while also expanding eCR and syndromic surveillance to additional sites. To address all of the above priorities, a greatly expanded information systems and informatics team will be established as a new Data Science Branch, led by a Data Officer (Branch Chief), supported administratively by a Systems Project Manager. The new branch will house additional informatics and IT staff including a Data Scientist, Data Engineer, Database Administrator, Business Analyst, Interface Administrator, and Epidemiologists to focus on syndromic surveillance and Big Data analyses. Develop systems to improve speed and efficiency of specimen submission to clinical and reference laboratories. b. Strengthen ability to quickly scale testing as necessary to ensure that optimal utilization of existing and new testing platforms can be supported to help meet increases in testing demand in a timely manner. c. Perform serology testing with an FDA EUA authorized serological assay in order to conduct surveillance for past infection and monitor community exposure. d. Work with LHDs to build local capacity for testing of COVID-19/SARS-CoV-2 including within high-risk settings or in vulnerable populations that reside in their communities. e. Apply laboratory safety methods to ensure worker safety when managing and testing samples that may contain SARS-CoV-2/COVID-19. Activity 1. Implementation Plan SLD will acquire high throughput instrumentation to support expanded molecular testing. The ThermoFisher TaqPath COVID-19 PCR is in the process of validation, and the Hologic Panther Fusion will be purchased in August. These platforms will increase the molecular testing capacity of the SLD from 200/day to 800-1000 tests per day. Other respiratory pathogens will be investigated for symptomatic patients not meeting PUI criteria, or samples negative for influenza or COVID-19. Additionally, MiSeq, nanopore, and Qubit are being acquired to establish Whole Genome Sequencing and thereby enhance epidemiological investigations. After the hiring a dedicated Biosafety Officer, SLD will strive to conduct outreach and work with testing laboratories to improve the climate of safety and encourage the conduct of site-specific and activity-specific risks assessments associated with COVID-19 testing. SLD will ensure safety precautions and proper building ventilation are in place. Access to testing has been limited through the pandemic to varying degrees and then often biased more toward some parts of the state's population and not others. Questions remain here as elsewhere as to the true prevalence of COVID-19 in our community. Additionally, there are questions regarding the role of certain populations (e.g., children) in driving disease. SLD plans to acquire two high throughput serological instruments to support serosurveys in select populations as determined by DOCD. DOCD plans to conduct serosurveys targeting certain populations and groups to determine the impact of COVID in these populations as well as their potential role in community transmission. Activity 1: Establish complete, up-to-date, automated reporting of morbidity and mortality to CDC and others due to COVID-19 and other conditions of public health significance, with required associated data fields in a machine readable format, by: a. Implementation Plan In order to bring Hawaii's morbidity and mortality reporting infrastructure up to date, the Electronic Communicable Disease Reporting System (ECDRS) will be redesigned and updated to accommodate the need for capture and storage of line-level laboratory reporting data and expanded set of required data elements in HL7 standard format. For validation of mortality reporting, there is currently a bidirectional interface between the electronic death registry system (EDRS) and Maven-based HI-EDSS. HDOH seeks to establish a similar interface for additional reportable conditions, including and most pressingly for COVID-19. The newly established Data Science Branch will work with the Vital Statistics Section to establish procedures and protocols for cross-referencing suspected and confirmed deaths due to reportable conditions other than influenza, starting with COVID-19. Implementation Plan Many laboratories establishing new test methodologies for detection of SARS-CoV-2 have been unable to quickly develop and send validated 2.5.1 HL7 messages to HDOH. Temporary solutions such as secure fax or csv extracts place significant burden on HDOH by requiring increased data entry time, slowing our ability to respond to new positive results, and jeopardizing capacity to completely document and deduplicate negative results. As an alternative to manual entry or batch CSV uploads, HDOH hopes to utilize the Maven Developer and Interface Administrator to translate csv data into HL7 xml files for importing into Maven. While ELC and ELC CARES funding supports eCR implementation and syndromic surveillance, much more robust staffing is needed to move eCR and syndromic surveillance implementation forward at the desired pace while also addressing unique informatics needs and challenges presented by the COVID-19 pandemic. At present, Hawaii has commitment from one major hospital as its pilot eCR participant, and our current PHINMS system is configured for receiving eCR messages both in test and production import feeds. HDOH has authorized COVID-19 Reportable Condition Knowledge Management System (RCKMS) in production, and has currently tested and reviewed sample COVID-19 eCR messages. The newly formed Data Science Branch will provide the additional informatics staffing needed to fully engage facilities, test and validate messages, and complete implementation of eCR and syndromic surveillance in Hawaii. Implementation Plan By enhancing our informatics workforce, HDOH hopes to accelerate our timeline for syndromic surveillance implementation, with influenza-like illness (ILI) and COVID-19-like illness (CLI) as top priority use cases. HDOH has already authored RCKMS for COVID-19 and plans to use this as one of the selected conditions for piloting eCR test messages from facilities. COVID-19 serves as a strong use-case and incentive for recruiting facilities to participate in these forms of surveillance. In addition to transmitting syndromic and eCR data to CDC, one of HDOH's objectives is to generate reports from data received from facilities to describe patterns and trends in ILI vs. CLI that can help with policy-making and planning. HDOH will also explore whether eCR can be used to to enhance existing sentinel surveillance systems for influenza and COVID-19 by identifying individual patients or groups who should be targeted for COVID-19 and other respiratory pathogen testing. Implementation Plan In order to bring Hawaii's morbidity and mortality reporting infrastructure up to date, the Electronic Communicable Disease Reporting System (ECDRS) will be redesigned and updated to accommodate the need for capture and storage of line-level laboratory reporting data and expanded set of required data elements in HL7 standard format. For validation of mortality reporting, there is currently a bidirectional interface between the electronic death registry system (EDRS) and Maven-based HI-EDSS. HDOH seeks to establish a similar interface for additional reportable conditions, including and most pressingly for COVID-19. We have already had to expand server capacity twice since the beginning of the pandemic. Issues that under normal circumstances might present a relatively minor inconvenience can become catastrophic, e.g., merge matching errors leading to large-scale deduplication failures with high volume of results reported for COVID-19. Epidemiologic priorities evolve rapidly in response to emerging policy questions and the demand for data to drive action, necessitating nearly weekly updates to our investigation question packages. Through the Maven Tier II Support hours, a designated Conduent Developer will work remotely based upon project requests. Projects will include mapping data to selected Maven screen templates (in particular, laboratory templates which are of interest for national reporting), and developing logic for conditions based on deduplication efforts. The MavenDeveloper will also be requested to assist with automating various data sources into Maven and can also be the primary programmer for custom Maven changes, extracts for message mapping guide messages, and bi-lateral data exchange between Maven and the Death Registry Program with COVID-19. Additional ITS Help Desk Support is needed to maintain system readiness and website functionality to maintain open access real-time data reports and visualizations to inform the COVID-19 response, and to set up equipment for the large number of newly onboarding staff. Various innovative digital and electronic tools have been and continue to be developed. We have already adopted one tool, HealthSpace, to enhance our capacity to monitor contacts such that investigators previously could manage monitoring ~5 contacts per day and now can manage at least 20 contacts per day. Implementation Plan The ILINet Sentinel Provider Recruiter will work to enroll more FQHCs from neighbor islands. DOH will coordinate with FQHCs enrolled as ILINet sentinel providers to conduct a seroprevalence survey. A protocol/plan will be developed with the FQHCs and HPCA (Robert Hirokawa). Specimen collection materials and transportation of the specimens to SLD for testing will be provided by DOH. Epi Specialists will follow-up cases that are antibody positive, and data will be analyzed by Epidemiologists. There are currently four sentinel providers enrolled in ILINet that serve homeless populations. These four providers are all on Oahu, but more providers who serve homeless populations are needed on neighbor islands. Homeless populations may have difficulty in accessing necessary healthcare and testing, which could translate to gaps in our surveillance. Providers will be recruited and onboarded so they understand how to report ILI on the online ILINet portal and how to send specimens to SLD for testing and surveillance purposes. The ILNet provider recruitment position will work with contacts from the Adult Mental Health Division (AMHD) to enroll these sites/providers. Hawaii is situated in the middle of the Pacific and is a popular tourist destination for travelers from all over the world. The Governor's 14-day quarantine requirement for all traveler arriving in the state effectively decreased not only COVID-19 introductions and activity but also that of other respiratory pathogens. As travel resumes, re-introduction of both COVID-19 and other pathogens is expected. Traveler health screening forms will be used for all incoming international and domestic travelers. These forms will screen for CLI symptoms, and travelers meeting symptom criteria will be asked to provide a swab for COVID and other respiratory pathogens testing at SLD. Such surveillance of travelers may offer some insight into the relationship of COVID-19 and other respiratory pathogens among travelers. Data collected from these forms will support disease investigations and contact tracing involving travelers; further, analyzed data may help inform future decisions on travel-related policies. Hawaii's state correctional and jail facilities are all chronically overcrowded, so introduction of any respiratory disease into these populations could prove disastrous. We have reached out to Public Safety Department colleage to establish a partnership that would facilitate specimens from symptomatic persons from various facilities being routed to SLD for testing for COVID-19 and other respiratory pathogens as part of our comprehensive respiratory pathogen surveillance. Maintaining relationships with acute care hospital infection preventionists is critical to maintaining situational awareness of the COVID-19 burden in our hospitals. Additionally, while COVID-19 is an urgently reportable condition in Hawaii, maintaining collaborative relationships with Infection Preventionists ensures all cases are promptly reported to HDOH which enables prompt contact tracing efforts and infection control outreach by the HAI team. As COVID-19 case response and follow up has monopolized the time of the existing HAI staff member, it has become increasingly difficult to proactively maintain positive relationships with infection preventionists. Having two dedicated HDOH-based IPs to maintain regular conference calls, create and disseminate updated guidance documents, and answer specific questions will ensure continued collaboration with our acute care hospitals in the future. While not the primary route to report COVID-19 cases to HDOH, NHSN will be a valuable tool to track COVID-19 trends across geographic areas as both acute care hospitals and long term care facilities are reporting COVID-19 cases to NHSN. This reporting route will also ensure no cases are missed, in the event a facility is delinquent in reporting via normal reporting routes, the CMS NHSN reporting mandate ensures this will be an intentionally duplicative reporting stream to ensure no cases are missed. Implementation Plan The SNF tele-ICAR effort is well underway, and by June 17, 2020, all 48 facilities will be completed. However, there is ongoing follow up required to close all identified infection prevention gaps, and continuous education and outreach is required now that we have developed relationships with all the facilities. Because the COVID-19 guidance is updated so rapidly, we require a team of LTCF-focused IPs to push out updated guidance to all the facilities and ensure everyone is staying up to date. Beyond SNFs, there are hundreds of ARCHs across the state, many of which have no or very little training in infection prevention skills. ARCH administrators often have English as a second language and are not comfortable using technology such as email, so they require special outreach efforts. The HAI team has been building relationships with the dialysis community over the last few years but requires now dedicated IP support to reach all the facilities consistently because of the particularly vulnerable nature of patients who require dialysis and who are also often residing in long term care facilities. Implementation Plan The HDOH HAI team enjoys productive relationships with community partners across the state such as the Healthcare Association of Hawaii (HAH), the John A. Burns School of Medicine (JABSOM), the Association of Professional in Infection Prevention and Epidemiology (APIC), and other professional organizations. We have longstanding relationships with the acute care hospitals and developing relationships with dialysis and long term care facilities. We intend to leverage these relationships to reach an even wider network of healthcare providers, so the community may understand the resources the HDOH HAI program has to offer, and we can partner to build IPC expertise statewide and in all variety of healthcare facilities. DOH will partner with all University of Hawaii (and potentially other universities) campuses across the state to conduct surveillance in their student populations. An existing ILINet sentinel provider from the University of Hawaii at Manoa Student Health Services may lead the expanded surveillance of all UH campuses. A protocol addressing data collection and specimen testing will be developed, and an MOA between DOH and UH parties will be signed. The possibility of a serosurvey of the student population at these campuses will also be explored. The epidemiologists in the Community Surveillance and Outbreak Response Unit will support the neighbor island epi specialists in their investigations of outbreaks in the community including schools and correctional facilities, and provide recommendations and guidance. Epi specialists will conduct outbreak/cluster investigations, public health educators will prepare education materials, prevention message, and other material for dissemination, and community health workers will translate the investigation findings into cummunity targeted messages. The Community Surveillance and Outbreak Response Unit will work with the epi specialists and the Policy, Education, and Outreach Unit to make sure the appropriate prevention messages are disseminated to the community.
