MANNER AND TIME FOR RESPONDING TO THIS NOTICE OF VIOLATION

Pursuant to HMC 4.07.060 (A), you must respond to the Notice of Violation within 30 calendar days
of the Date the Notice of Violation was served by doing one of the following:

1. If you are not disputing the violation, submit payment of the Civil Penalty Amount
with a copy of this Notice of Violation to the following: City of Henderson, Attn:
Business Operations Division, 240 Water Street, PO Box 95050 M30 121,
Henderson NV 89009-5050. (Payment plans or arrangements may be requested as
set forth below).*

OR

2. If you are disputing the notice of violation pursuant to HMC a
hearing may be requested by sending a written request to the following: City of
Henderson, Attn: Business Operations Division, 240 Water Street, PO Box
95050 M80 121, Henderson NV 89009-5050. The request must include the
following information:

a. The name of the person or entity responsible for the

Violation;

b. The current address of the person or entity responsible for

the Violation; and

c. The Notice(s) of Violation(s) which the person wishes to

dispute at the hearing.
After receipt of a person's written request for a hearing, the division shall set a
hearing date and provide notice of the hearing to the person requesting it at least 14
days before the date of the hearing.

Failure to respond to this Notice of Violation within the time described above may result in an additional
civil penalty in the amount of the original civil penalty amount. Providing an incomplete or incorrect
address in connection with a hearing request shall be considered the same as not responding to this
Notice of Violation.

Pursuant to 4.07.070, "ln addition to the civil penalties, the City will assess additional fees for the collection
of the penalties. The amount of such fee shall be 25 percent of the outstanding indebtedness or $250.00,
whichever is less. The amount of any such collection fee shall accrue and become due and payable at the
time the indebtedness is referred for collection to the collection agency, and that amount may be added by
the collection agency to the amount sought to be collected. Any judgment or amended judgment entered
under this chapter may include the amount of the collection fee authorized herein."

*Payment Arrangements:

If you do not dispute the violation(s) but need to request a payment arrangement to pay the Civil Penalty
amount, please submit a written request to the following: City of Henderson, Attn: Business Operations
Division, 240 Water Street, PO Box 95050 M50 121, Henderson NV 89009-5050. The request must
include the following information:

A copy of this Notice of Violation; and
A written request for payment arrangements detailing the amount to be paid and time frame to be paid.
(Note: Proposed payment plans in excess of six months will generally not be permitted).

The request for payment arrangements must be received by the Division within 15 calendar days of the
Date the Notice of Violation was served. The request will be reviewed by the Division and is not
guaranteed. lfthe request is not granted, the person or entity who made the request is still responsible to
comply with the ?Manner and Time for Responding To This Notice of Violation" as noted above.

Note: The civil penalty assessed is separate and in addition to any license fee, late penalty,
reinstatement fee, etc. that may have been assessed on your license. Payment of said feels) after
the assessment of the civil penalty does not remove the obligation to pay the civil penalty once
assessed.
