This project was approved by our office in 1998 In 2007, DSA closed this file without certification. DSA District Engineer (R. Potter) site visit on 3/l 1/99 disclosed the following: On North side of structure an extremely large fireplace and chimney dominates Lhe wall, The stone appears to be urireinforoed and appear to be reasonably atiached to the structure. Even the chimney poriion is about 2 ft. nhick and 3 ft. wide. Due to the extremely poor record of Lmrehifcrced rock chimneys collapsing even in minor earthquakes the chimney should be assessed and strengthened ifnecessary. 2. Review ofthe modernization plans io: the project indicates a previous DSA approval number of 3084 well before appropriate seismic design existed. Because the East wall of the structure has 7 iurge penetrations with thin walls at each end of the wall this detail should be looked zz for seismic adequacy. R. Potter cc. However, Verified Report documents and/or affidavits required to complete the file have not been received from the responsible entities, and the Division ofthe State Architect held engineers final recommendations indicate potential dehciencies which are listed below: FIELD TRIP NOTE #1 IDENTIFIED AN EXISTING SEISMIC HAZARD THAT REQUIRES RESOLUTION PRIOR TO CLOSE WITH CERTIFICATION. IN ADDITION THE FOLLOWING DOCUMENTS REMAIN OUTSTANDING: THE DSA 5 BUILDING INSPECTOR QUALIFICATION RECORD FOR THE PROJECT INSPECTOR, CONTRACT INFORMATION FORMS, FINAL VERIFIED REPORTS FROM THE ARCHITECT, FINAL VERIFIED REPORTS FROM THE INSPECTOR OF RECORD, FINAL VERIFIED REPORTS FROM THE LABORATORY FINAL AFFIDAVIT, THE NOTICE OF COMPLETION FROM THE SCHOOL DISTRICT. As these deficiencies remain uncorrected, the Department of General Services cannot certify this project. No further action by our ottice is anticipated on this project, and regrertully the file is being closed without issuance of the Departments certification of construction. To reopen the project file, a submittal of outstanding documents along with a warrant for $150 is required to recover our costs It the listed documents cannot be obtained, an explanation regarding the attempts to secure them may be considered.
